those members of the community who are without medical benefits.
We promote better life choices through education and preventive
care to foster the growth of the whole individual.

City Health Partnership is staffed by dedicated physicians,
nurses and other local area healthcare professionals, who volunteer
their time to provide quality care to the community. Our services
are for low-income residents of central Kane County who do not have
Medicare, Medicaid/Public Aid or who are underinsured.
You can find us in downtown St. Charles near the intersection of Route
31 (Geneva Road) and Route 64 (North Avenue).
